G3061hiresWe provide a low to medium floating support service to service users in their home. Support provided depends completely on the individual and can work in accordance with care plans produced by other agencies working with an individual. Our support plans take in to account service users’ goals and aspirations in the planning and management of service delivery. This results in the creation of individual support plans that may include culturally specific services.

Service users are expected to be visited 3-4 times a week for an average period of 45 mins-1 hour per visit. Shorter or longer contact is available depending on the circumstances at the time. Contact by phone is also encouraged where necessary with a minimum of 2 calls a week made by a service user’s support worker. This contact enables support officers to monitor and look for change and suggest actions alongside with other appropriate services.

Support can be provided for up to 2 years.

Support includes:

  • Encouraging service users to develop and learn new skills and where necessary support access to resources available. Skills may include budgeting and household management (promoting the payment of rent, or emotional management.
  • Providing practical support including advocacy with welfare benefit issues.
  • Housing support, including assisting service users move in or out to more appropriate accommodation.
  • Ensure service users are able to fully access local services in accordance with their needs and to ensure effective communication is available and maintained with Health services.
  • Monitoring service user’s physical and mental wellbeing and liaising with other agencies where necessary.
  • Supporting medicine management prompting medication where necessary.
